Welcome to Saint Elizabeth Health Care, where we bring health care services to you. We recognize that people value quality, convenience and access to care, which is why SEHC has thrived for more than a century by providing health care in many different settings. We have experience working in urban, suburban, rural, remote and developing communities – from downtown Toronto to northern Manitoba and as far away as Trinidad and Tobago! So whether you are in a living room, boardroom or room on wheels, you can be assured our care will be tailored to your needs as well as your location.

Saint Elizabeth Health Care provides a broad spectrum of services that allow people to live in familiar surroundings, attend school, maintain employment and participate in other valued activities. The focus of home care services extends beyond the immediate medical issue to include health promotion, illness prevention and the proactive management of one’s condition and environment in order to prevent emergency room visits, hospital admissions or transfers to residential care facilities. In addition to allowing clients to remain where they feel most comfortable, home care enables you and your family to play an active role in your care, while responding with sensitivity to your individual needs and preferences.

Clinics

Clinics provide a timely, flexible and efficient option for care, allowing clients to schedule visits at their convenience while optimizing the use of health human resources and helping to ease pressures on hospital emergency departments. In collaboration with Community Care Access Centres, Saint Elizabeth Health Care has extensive experience delivering nursing and therapy services in ambulatory and community-based clinic settings throughout Ontario. Services range from the delivery of intravenous infusions to the management of complex wounds. We also offer a range of mobile/flex clinics for under-housed and marginalized populations.

Community Care

Saint Elizabeth Health Care provides innovative programming and support services in a range of community environments, including:

  • Schools
    In collaboration with Ontario’s Community Care Access Centres, SEHC provides school-based health care services to children who otherwise would be unable to attend school, or whose education would be disrupted by having to leave the school setting for required care and therapy. School health services aim to support a child’s independence and maximize their ability to participate in educational activities and instruction.
  • Alternative Housing
    SEHC works in partnership with other community organizations to meet the health care needs of homeless, under-housed and marginalized populations through innovative models of service delivery including care in shelters and mobile/flex clinics.
  • Community Crisis Response
    Saint Elizabeth Health Care provides integrated mobile crisis services that are built on a network of community linkages and partnerships. We offer timely mobile crisis visits in the home or community as well as 24-hour crisis intervention and support by phone.

Hospices

Hospice is about life and helping people to live fully every day. SEHC is committed to caring for people at their greatest time of need and works in partnership with other organizations to provide deeply compassionate hospice care to those living with a life-threatening illness. Through a unique model of integrated care delivery, our nursing, personal support and therapy services are embedded into the broader health care team of the hospice to support residents in their last phase of life. 

Other Facility-based Care

Saint Elizabeth Health Care collaborates with hospitals, long-term care facilities, complex continuing care and retirement residences to provide a range of direct care, education and staff relief services in institutional settings. Our areas of specialty include but are not limited to: dialysis, palliative care, oncology, maternal/newborn, cardiorespiratory care including home ventilation, post-surgical care, physiotherapy and rehabilitation services and general medical care. Through numerous community-based contracts, we have a track record of working collaboratively with multidisciplinary facility-based teams and ensuring compliance with policies, procedures and documentation systems as required.

SEHC also provides personal support and homemaking services to eligible senior residents of supportive housing developments. A dedicated team of staff is assigned in order to achieve high levels of continuity of care.

Virtual and Technology-assisted Care

Saint Elizabeth Health Care is leading the industry in its incorporation of technology to improve the client experience and advance best practices in health care. For example, we were one of the first organizations in Canada to trial the C5 Tablet computer. With the tablet and our technology-assisted wound consultation tool, a nurse can take a photograph and complete a detailed wound assessment and then send it wirelessly to a wound specialist for consultation – at any time and from any where.
