Personal/Health Information Privacy Commitment and Policy

At Saint Elizabeth Health Care, your privacy is important to us. To help you understand how Saint Elizabeth Health Care protects clients’ confidential personal/health information, we have developed the following Personal/Health Information Privacy Commitment document.

This Privacy Commitment is Saint Elizabeth Health Care’s commitment to you, as a valued client, that we will protect your personal/health information by adhering to the following principles:

  • Saint Elizabeth Health Care staff will safeguard, according to strict standards of security and confidentiality, any information you share with us.
     
  • Saint Elizabeth Health Care will limit the collection, use and disclosure of client information to what is necessary to deliver comprehensive health care services or to administer our home health business for clients.
     
  • Saint Elizabeth Health Care will only permit its authorized employees – those who are either providing direct health care service or providing administrative support – to have access to client information. In summary, only those staff who have a need to know or a right to know will have access to client information. This will apply to information in all forms – paper client charts, administrative forms, and electronically-based information.
     
  • All Saint Elizabeth Health Care employees will sign an individual confidentiality and privacy commitment upon employment. This commitment will be reviewed on a regular basis. All employees will undergo rigorous privacy education and regularly review privacy policies and procedures. Employees who violate our Privacy Commitment will be subject to our normal disciplinary process.
     
  • Saint Elizabeth Health Care will not reveal client information to any external organization unless we have previously informed the client in disclosures or agreements, have been authorized by the client, or are required to do so by law.
     
  • Whenever Saint Elizabeth Health Care hires other organizations to provide support services, they will be required to conform to our privacy standards and to allow us to audit them for compliance.
     
  • Saint Elizabeth Health Care will not use or share – internally or externally – personally identifiable medical information for any purpose other than the provision or administration of a client’s home health service or account, or as disclosed to the customer when the information is collected, or to which the customer consents.
     
  • Saint Elizabeth Health Care will strive to keep all client files complete, up-to-date and accurate. Saint Elizabeth Health Care will advise clients how and where to conveniently access their health record information and how to notify us of errors, which will be promptly corrected.
     
  • Saint Elizabeth Health Care has appointed a Privacy Officer to oversee, monitor and report all client privacy issues and concerns. You may contact the privacy officer by:

Accessibility Commitment and Customer Service Standard

At Saint Elizabeth Health Care, we are committed to ensuring that our goods and services are accessible to all people, including individuals with disabilities, in alignment with our vision to honour the human face of health care. To help you understand how Saint Elizabeth Health Care upholds the core principles of independence, dignity, integration and equality of opportunity in our interactions, we have developed the following Accessibility Commitment and Customer Service Standard document.

This Accessibility Commitment is Saint Elizabeth Health Care’s commitment to you, our valued client and/or customer, that we will provide goods and services in an accessible manner by adhering to the following principles:

  • Saint Elizabeth Health Care will accommodate the use of assistive devices or any alternative measures used by people with disabilities to access the goods or services provided by our organization.
     
  • Saint Elizabeth Health Care will take a person’s disability into account when communicating with the individual.
     
  • Saint Elizabeth Health Care will allow people with disabilities to keep their service animals with them on the parts of our premises that are open to the public or other third parties, except where the animal is excluded by law; in cases where a service animal is excluded by law from our premises, SEHC will ensure that alternate measures are available to enable the person to access our goods or services.
     
  • Saint Elizabeth Health Care will permit people with disabilities to enter the parts of our premises that are open to the public or other third parties with their support person, and provide notice of any fee charged for the support person, where applicable.
     
  • Saint Elizabeth Health Care will post a notice at a conspicuous place on our premises, on our website or by another reasonable method, of any temporary disruption in access to facilities, goods or services, including the reason, duration and any alternatives available.
     
  • Saint Elizabeth Health Care will provide a documented process to receive and respond to feedback on how our goods or services are provided to people with disabilities, including actions we will take when a complaint is received.
     
  • Saint Elizabeth Health Care will make information about our feedback process readily available to the public, including how feedback may be provided.
     
  • Saint Elizabeth Health Care will ensure that our health care service providers, third-party suppliers and staff members who deal directly with the public and/or participate in the development of SEHC policies and procedures, receive training about providing goods and services to people with disabilities. This training will include current policies, practices and procedures outlined in this Accessibility Commitment and Customer Service Standard.
     
  • Saint Elizabeth Health Care will maintain a written training policy and record including a summary of our training content, schedule and registrants.
     
  • Saint Elizabeth Health Care will make documents available upon request in a format that takes a person’s disability into account.
