Enhancing support and education in the delivery of home health care

Caregiving Guides
In an effort to enhance the support and education available to family caregivers, the Saint Elizabeth Health Care Foundation has sponsored the printing and distribution of two illustrated, easy-to-follow guides, Caring for Loved Ones at Home and Family Hospice Care.

The Foundation's charitable contribution to publish, print and distribute these books made the largest dissemination of palliative care information possible in the history of Ontario, for which we were named Philanthropist of the Year in 2000 and again in 2009 by the Ontario Palliative Care Association. To date, over 200,000 copies of these books (a value of over $4.6 million) have been distributed, free-of-charge, to help caregivers effectively manage and respond to the home care needs of their loved ones.

@YourSide® Caregiver Support
@YourSide Caregiver Support is an interactive, web-based solution designed to support and assist caregivers, including family members, friends, neighbours and volunteers. Developed by the health experts at Saint Elizabeth Health Care, key features of the program include:

• Comprehensive caregiving information, tips and advice that can be customized to meet individual learning needs
• Monitoring tools for caregivers to record and track their own health and wellbeing, as well as those of their loved ones
• Online appointment calendar
• Answers to common questions
• Links to other helpful websites and additional resources
