Meet SEHC

The Saint Elizabeth Health Care family consists of six dynamic structures united by a common vision to honour the human face of health care. Together, we provide a full range of integrated care solutions that creatively respond to client, community and health system needs across Canada and beyond.

Continuum

Continuum, a division of Community Rehab, provides community based services to individuals suffering from automobile, illness, slip and fall and/or work-related injuries. Our early intervention and ongoing rehabilitation services assist people with physical, psychological or environmental barriers that prevent them from returning to their former level of function. Acquired and Traumatic Brain Injury Rehabilitation is one of Continuum’s specialized areas of practice. Our therapists assist those recovering from brain injuries reintegrate into their home, school, work and community environments. Continuum also provides a full range of in-home and on-site rehabilitation services to facilitate the transition from hospital discharge to community reintegration. The Care to Know Centre

The Care to Know Centre (CTKC) is the national knowledge exchange network of Saint Elizabeth Health Care. Powered by knowledge networks and social innovation, the CTKC is founded on a collaborative approach that seeks to mobilize input and to create effective linkages and exchange amongst individuals, caregivers, researchers, policy and management stakeholders. Partnering directly with those that provide and receive care, for the purpose of research, is an essential way to improve and influence client health experiences. The Care to Know Centre’s ACT Research Grants program is yet another example of Saint Elizabeth Health Care’s commitment to continually strive for the best outcomes possible for clients across the health continuum.

Community Rehab

Forward-thinking and people-focused, Community Rehab is an interdisciplinary Canadian home health care organization that has been a leader in rehabilitation since 1985. In 2008, Community Rehab and Continuum joined the Saint Elizabeth Health Care family and continue to operate as an independent organization focused on rehab excellence, innovation and collaboration. With five offices throughout Ontario and a dedicated team of 300 therapists, including occupational therapists, physiotherapists, dietitians, social workers and speech language pathologists, Community Rehab delivers more than 200,000 therapy visits to approximately 35,000 clients annually. Saint Elizabeth Health Care Foundation

Building on a solid foundation, Saint Elizabeth Health Care established a separate charitable not-for-profit foundation in 1997 to help lead community health care into the 21st century. Inspired by the mission of SEHC, the Foundation is dedicated to raising funds for the advancement of knowledge and charitable initiatives in home and community care. Core charitable programs include caregiver support, chronic disease self-management, and education and support for health care providers in First Nations communities. The Foundation was the proud recipient of the Ontario Palliative Care Association’s Outstanding Philanthropist Award in 2000 and again in 2009, in recognition of its exceptional financial support and outstanding civic and philanthropic leadership.

ElderSafe Support Services

ElderSafe Support Services, a division of Saint Elizabeth Health Care, has been providing the highest quality non-medical home health care and support to Canada’s growing seniors’ population since 1995. With locations in British Columbia and Ontario, ElderSafe offers a full range of customized elder care services including personal care, companionship, housekeeping, post-surgery care, dementia and Alzheimer care, respite care, palliative care, 24 hour assistance, light meal preparation and transportation services.
