Home and community care encompasses a broad range of health services delivered in the home and throughout the community to recovering, disabled, chronically or terminally ill persons in need of medical, nursing, social or therapeutic treatment and/or assistance with the essential activities of daily living.

Today almost any type of care offered in an institutional setting, short of surgery, can be provided in the home environment. Home care services may include:

  • Nursing
  • Personal support (i.e. assistance with bathing, dressing, feeding, etc.)
  • Physiotherapy
  • Occupational therapy
  • Dietetics
  • Social work
  • Speech therapy
  • Homemaking
  • Caregiver respite (relief)

Home care programs can also provide linkages to other community resources such as Meals on Wheels and transportation services, etc.

You can access home care services through the publicly-funded health care system by contacting your local government home care program (see list below). Additional care and services are also available through private payment, personal or group insurance plans, and charitable programs. To discuss your needs and options, please contact the regional director at your local Service Delivery Centre.

Selecting a home care provider is an important decision, and like most things in life, doing some basic research beforehand will help ensure the choice you make is the right one for you. At SEHC, we believe an informed client is the best possible type of client to have.

Some questions you might want to ask include:

  • What kind of services do you provide?
  • Are your services available 24 hours a day, 7 days a week?
  • How much do your services cost and what funding options are available?
  • Who will be involved in my health care team?
  • Will I be included in the development of my care plan?
  • Can you accommodate special language requirements?
  • How do I request a change to my care plan, visit schedule or care provider?
  • How long have you been in business?
  • Is your organization accredited by Accreditation Canada?
  • Do you conduct background and criminal reference checks before hiring?
  • Are your staff certified by their respective professional colleges?
  • Do your staff receive additional education and training?
  • Will your staff reassess my needs on a regular basis?
  • These factors will help you determine the quality of services you can expect to receive from a provider, as well as their philosophy regarding your care.
